Which game are you referring to? If you're asking about Dweep Gold, a decoy item just means an item that you shouldn't pick up -- if you do, you can't finish the level. For instance, if there's a torch surrounded by ice, that's probably a decoy, since you'll waste two torches trying to get it (one to get in, and one to get out). Decoys are a design trick; using decoys makes a level more challenging, since players will naturally want to pick up every item.

In Dweep Gold you place a decoy item the same way you place a regular item. The only difference is how you design the level. If you design the level a certain way such that a particular item shouldn't be picked up, then you might consider that item a decoy. So decoys aren't a special feature of the level editor. Decoys are a level design trick.
